Liability Release and Acknowledgement of Risk
I, _________________________________, hereby acknowledge that I have
voluntarily applied for the services of Gypsy Oak Retrievers to train my dog.
As lawful consideration for being permitted to participate in this
program, I hereby agree that I, my heirs, distributees, guardians, legal
representatives, and assigns will not make a claim against, sue, attach the
property of, or prosecute Gypsy Oak Retrievers, Lindsay Robinson, any member,
agent, employee, or contractor of Gypsy Oak Retrievers as a result of my
participation in this activity.
I understand that there are risks involved in training dogs that could
result in physical injury or even death to me, my family members and/or my
animals.
THE RISKS INCLUDE, BUT ARE NOT LIMITED TO: the use of firearms, contact
with wild animals, running on uneven terrain, swimming and exposure to the
elements, latent or apparent defects or conditions of equipment, transport and
travel.
Furthermore, I understand that dogs can and do get sick, injured and
become ill. In the event my dog requires medical attention, I give my permission
for my dog to be taken to a certified Veterinarian for treatment. I also
understand that I am fully responsible for all expenses related to said
treatment.
I have read and understand this Release and enter into this agreement
by my own free will and choice on this ____ day of _______________, 20___.
